Audit trail
An immutable record of who did what, when, and on what basis, kept alongside the case itself.
An audit trail is the answer to the question an inspector, an auditor, or a court eventually asks: how was this decided? It records actions, actors, timestamps, and the data the decision was based on, in a form that cannot be quietly edited afterwards.
The reason to generate it automatically is not diligence but reliability. Trails that depend on someone remembering to write a note are incomplete precisely in the cases where completeness matters.
